Open Records Request
The Morgan County Communications Center only processes requests for our agency. If you are requesting records managed by the Morgan County Communications Center, please download the Morgan County Communications Center Open Records Policy and Form and HIPPA Form and either email the completed form back to us or return it to the Morgan County Communication Center located at 400 Warner St., Fort Morgan, CO.
Individuals / Patients: Please complete the HIPAA Release Form included in the link above.
- The form must be filled out completely and must be returned to the Morgan County Communications Center with a copy of a valid state issued ID. No records will be released without a complete form and a copy of a valid ID.
